    Student Information Form

    NOTE: The information below must be provided for each student attending the dance. If you have multiple students attending, please complete a separate form for each student. This information must be completed in full for tickets to be processed.

    The welfare and safety of all participants at the 2019 Gold & Silver Ball is achieved only with your knowledge and participation in this responsibility. Please review these guidelines with your child(ren) and both sign as indicated.

    • Any child who arrives under the influence, suspected to be under the influence, or in possession of alcohol or drugs will not be allowed to attend the dance, and the parent will be immediately notified. The parent (or responsible adult) will be expected to pick up the child from the dance as soon as possible. The child will be looked after by a staff member/chaperone until a parent/responsible adult arrives.
    • In the event of an emergency, if we are unable to reach a parent or responsible adult, the child will be taken by ambulance to the hospital for medical care.
    • Guests will be checked on arrival. Any and all bags, of whatever nature, will be inspected.
    • The Committee strongly suggests that parents do not send their child in an un-chaperoned limousine, particularly if coming from outside the city. Please make sure that the driver waits until your child has been admitted and that your child has the limousine company’s telephone number and a number where a parent can be reached that evening. Also check that the limousine does not have a stocked liquor bar.
    • The Bone Marrow & Cancer Foundation is not responsible for any lost or stolen items.
    • The dance ends promptly at 12 midnight. The Bone Marrow & Cancer Foundation cannot assume responsibility for transportation. Please make prearranged plans with your child.
    • Neither the Committee nor the Bone Marrow & Cancer Foundation is responsible in the event that any accident or injury occurs at the dance.
    • Students are not permitted to re-enter the dance; all exits from the venue are final.
